Choose your order method

Order online: Easy, paperless test ordering & management

Our online ordering portal, Nucleus, provides a fast, efficient, secure, and HIPAA-compliant way for healthcare providers to order tests and access their patients’ test result. Browse our test catalog to select a curated panel or create a custom test. Save your order at any time to return to it later.

Order on paper

If you prefer to order via paper, download the requisition, complete the required information, and send the requisition form to us by land mail or fax. After the referral form has been received and processed, you will receive an order confirmation email with your login details for the Nucleus portal. You can then use your login to place new orders, follow the status of your orders, and access the statements of your patients with completed tests.

Please be advised that we only accept specimen collection kit requests from medical professionals. If you are a patient or family member of a patient, please contact your provider to place a kit order on your behalf.

You can order up to 10 kits per type and 30 kits total. If you need more kits, please contact our customer support team.

At this time, we are unable to process any orders from New York State, outside of sponsored testing. Please note Familial Variant Testing (FVT) and Targeted Variant Testing (TVT) are also not available at this time to New York patients, even following use of a sponsored test. We apologize for any inconvenience this may cause clinicians or patients in New York State. You can order kits for sponsored testing (i.e. NovoDETECT, My Retina Tracker, and Resonate Program) below. You can order a package including only buccal swab devices (ORACollectDNA® OCR-100) and tube labels. Devices are sent in multiples of 25 and the service is dedicated to high-volume customers. Please note that shipping materials are not included in the package. Shipping fees are covered by Blueprint Genetics when using our courier and provided account number. We are happy to assist in the shipping process.
